I, certify that I have been authorized to execute this agreement on behalf of the before mentioned Organization to purchase an advertisement in the Mt. Lebanon Baptist Church 100th Anniversary Commemorative Journal. I furthermore agree to deliver a digital specification-ready copy of the advertisement and/or patron ad. Advertisements and forms not received by the deadline, will not be included in the journal. Mt. Lebanon Baptist Church - Baltimore reserves the right to refuse publication of any advertisement deemed inappropriate.
All rates are non-negotiable, cancellations are not permitted and payment in full is required for publication.
